Neighborhood Revitalization 2022
Modernize Clem Dufour Park to better serve resident gatherings sponsored by Habitat for Humanity including computer classes for residents of all ages, financial health workshops, and a Kid’s club which provides reading and literacy supports for Pre-K – 5th grade.
Peer Navigation for Weld County Veterans
Employment and community reintegration services including mental and behavioral health resources, spousal counseling, and substance abuse counseling for veterans.
Positive Futures for Dreamer Scholars at Frederick High School
Academic, social-emotional and career path support for under-resources youth and their families at Frederick High School.
SEMH Programming for Positive Futures for Dreamer Scholars
Life skills training, leadership skills, identity exploration and conflict resolution for socio-economically disadvantaged students.
Southern Weld County Rapid Rehousing Project
Rental assistance, move-in costs, and case management expenses for 12 households experiencing homelessness to provide rehousing support.
Suicide Education & Support Services in Weld County
Crisis intervention and other support services to survivors of suicide. Additionally, trainings for middle and high school students, businesses and other places in the community to teach how to recognize the warning signs of suicide, know how to ask for and give help, and know the resources available to individuals and their loved ones.
Support Early Literacy and Healthy Relationships for Weld County Kids
Developmentally appropriate books in up to 30 languages for 10 Weld County partner clinics to be distributed to children during wellness visits. The program will impact over 10,500 children, including approximately 1,876 newborns and 1,000 children from a new partner clinic.
