Hope House Teen Mom Access to Resources 2023
GED testing fees and materials, certification and college admission fees, financial literacy classes, as well as emergency car repair support for teen moms to guide them to self-sufficiency. The program also provides ASQ-3 evaluations to assess children’s developmental milestones to refer for interventions if necessary.
Hospital to Housing Program 2023
Follow-up care (including discharge directives, appointments and therapy referrals), job search support, connection to community resources and transition to sustainable housing for patients discharged from the hospital without stable housing or family to care for them.
